1. Applications must be received on this form and address all questions and selection criteria.
2. The QV Healthcare Practitioner Student Scholarship is administered, funded and awarded by Ego Pharmaceuticals Pty Ltd (ABN 86 005 142 361) of 21-31 Malcolm Rd, Braeside VIC 3195 (‘Ego Pharmaceuticals’). Employees (and their immediate families) of Ego Pharmaceuticals are ineligible to enter. Immediate family means any of the following: spouse, ex-spouse, de-facto spouse, child or step-child (whether natural or by adoption), parent, step-parent, grandparent, step-grandparent, uncle, aunt, niece, nephew, brother, sister, step-brother, step-sister or 1st cousin.
3. The scholarship is open to any Australian resident who is currently enrolled and participating in a tertiary course in the fields of medicine, pharmacy or nursing. The scholarship is not affiliated with any tertiary institution.
4. Applicants must complete a short essay answering the question: “How will you utilise your field of study and the $10,000 scholarship prize to benefit patients with sensitive and problematic skin?”. There is a word limit of 250 minimum and 500 maximum words. Applicants must detail how the $10,000 scholarship award will be applied against their ongoing study (such as undertaking further study, training or conference attendance in Australia or overseas).
5. Ego Pharmaceuticals reserves the right to withdraw and/or not award a scholarship in any year.
6. Ego Pharmaceuticals will undertake assessment of all applications and their decision will be final and no correspondence will be entered into.
7. Late or incomplete applications will not be accepted.
8. There will be up to three (3) successful applicants, who will each receive the sum of $10,000 that is to be used for further study, training, conference attendance etc. Successful applicants are required to provide an Australian bank account details for payment of the scholarship sum. Payment shall be made within 30 days of announcement of successful applicants.
9. Successful applicants may be required to provide a brief biography and picture of themselves. Successful applicants are required to submit a brief report to Ego Pharmaceuticals within nine months of being awarded the scholarship on how the scholarship has assisted with their studies. Ego Pharmaceuticals should be acknowledged in any publication, social media post or presentation resulting from the scholarship award.
10. Ego Pharmaceuticals reserves the right to publish information about the scholarship and successful applicants in their medium of choice.
11. Applicants should take their own advice in relation to tax implications of receiving a scholarship award.
12. Any costs associated with applying, entering or participating the scholarship program (including internet access and download charges) will be at the cost of the applicant.
13. Applicants agree to their entry being used by Ego Pharmaceuticals in the future and grant Ego Pharmaceuticals a non-exclusive, royalty-free, perpetual, worldwide, irrevocable and sub-licensable transferable licence to use, reproduce, modify, adapt, publish and display the entry for any purpose in any media without compensation, restriction on use, attribution or liability. Applicants warrant that they have full authority to grant these rights and will not assert any moral rights in respect of the entry. Ego Pharmaceuticals collects personal information (‘PI’) in order to conduct the scholarship program and may, for this purpose, disclose such PI to third parties, including but not limited to agents, and, as required, to Australian regulatory authorities. Entry is conditional on providing this PI. Ego Pharmaceuticals will also use and handle PI as set out in its Privacy Policy, which can be viewed here. In addition to any use that may be outlined in the Ego Pharmaceuticals’ Privacy Policy, Ego Pharmaceuticals may, for an indefinite period, unless otherwise advised, use the PI for promotional, marketing, publicity, research and profiling purposes, including sending electronic messages or telephoning the applicant. The Privacy Policy also contains information about how entrants may opt out, access, update or correct their PI, how entrants may complain about a breach of the Australian Privacy Principles or any other applicable law and how those complaints will be dealt with. All entries become the property of Ego Pharmaceuticals. Ego Pharmaceuticals will not disclose PI to any entity outside of Australia.
14. By submitting an entry on the entry page, applicants confirm that the short essay is original work that they have undertaken independently and that all information submitted on the entry form is accurate and an applicant will submit substantiating information if required by Ego Pharmaceuticals. Applicants consent to receive marketing communications from QV and Ego Pharmaceuticals from time to time (unsubscribe at any time).
